- Benefits vary, but a typical plan offers a low $50 deductible, 100% preventive care, 75% basic care, and 50% major care benefits up to an annual maximum of $1,000 per covered person per calendar year.

Term Life Insurance With Accidental Death & Dismemberment
- Available with or without health insurance to groups of two or more employees
- Up to $100,000 of coverage per employee ($50,000 if included on health insurance plan)

Short Term Disability
Help prepare for accidents and the unexpected!
- Benefits paid on the first day of an accident or the 8th day of an illness, for benefit period options of 13 or 26 weeks, with a maximum weekly benefit of $750
- Available without health insurance to groups with two or more employees (not available with health insurance plan)
Long Term Disability

- Available to groups with 10 or more employees
- Benefits paid at 60% of salary up to a maximum of $6,000 a month to age 65
